2023-2024 Registration

Please download any forms you need. If you use the fillable PDF format, you can email registration packets to [email protected].  Paper registration packets can be picked up by calling the high school office at 217-636-8314. Optional and Medical forms will have to be downloaded and printed or requested from the high school office. 

 

Parents/Guardians of returning students can pay online through Skyward at https://athens-213.revtrak.net/ after May 1.  Please make sure you choose Athens High School and the 2023-2024 button. A processing fee of $3 is added to the registration cost when using this option.  You will also be able to pay your student’s lunch account, drivers education and sports fees through this method (there is no additional processing fee for lunch accounts, drivers education or sports fees).

 

ALL INCOMING FRESHMEN MUST HAVE A PHYSICAL, REQUIRED IMMUNIZATIONS AND DENTAL EXAM.

 

NEW STUDENTS FROM OUT OF STATE MUST HAVE A PHYSICAL, REQUIRED IMMUNIZATIONS, DENTAL EXAM AND EYE EXAM. 

 

ALL ATHLETES MUST HAVE A CURRENT SPORTS PHYSICAL BEFORE THE FIRST PRACTICE.

 

ALL SPORTS FEES MUST BE PAID BEFORE THE FIRST GAME IN THAT SPORT.

 

MEDICAL FORMS (have to be downloaded and printed or requested from the office): IHSA Sports Physical, School Physical, Dental Exam, Dental Exam Waiver, Eye Exam, Eye Exam Waiver, State Health Requirements for 2023-2024, Over the Counter Medication Consent, Request for Prescription Medication at School, Self-Administration of Specific Medications at School. 

 

OPTIONAL FORMS (have to be downloaded and printed or requested from the office): Student Parking Permit Registration, CACC Driving Waiver, Reduced/Free Lunch Application (available in July), Reduced/Free Registration Application, Pesticide Registry
